The MO HealthNet Renewal Form Is Missing. Find the Paper Trail Before Coverage Closes

A missing renewal form can become a coverage loss, so confirm the due date, identify the form FSD expects, and preserve proof of every submission before you assume the case renewed itself.

The papers ·

You know MO HealthNet coverage exists because you have the approval letter, the card, or paid claims. What you do not have is the annual renewal form. Do not treat an empty mailbox or an empty portal as proof that nothing is due.

Local option: New Plan Care gives families in the St. Louis area in-home help so a parent can stay home: companionship, help with bathing and dressing, meals, and overnight care. Call (314) 405-0887.

The early warning signs are ordinary: the participant moved, mail still goes to an old address, the portal shows no renewal, two household members have different due dates, or a provider suddenly cannot confirm eligibility. Your first job is not to complete a form you cannot see. Your first job is to make the Family Support Division, or FSD, identify the case, the renewal due date, and the paper it expects.

Who pays

MO HealthNet pays covered claims only while the participant remains eligible and the coverage record is active. FSD conducts the eligibility renewal. A doctor, hospital, pharmacy, managed care plan, or Medicare does not complete that renewal for the participant.

If the person also has Medicare, keep the two files separate. Medicare coverage does not prove that MO HealthNet remains active, and a Medicare card does not answer whether MO HealthNet will pay deductibles, coinsurance, premiums, or Medicaid-only services. The federal Medicare materials and Missouri DSS materials collected on the Sources page describe different programs with different records.

Before paying a bill that appears after a suspected closure, ask the provider to state which payer rejected the claim, the date of service, the rejection reason, and whether the claim can be resubmitted if eligibility is corrected. Do not accept “Medicaid is gone” as the complete answer. You need the payer name and the rejection code or written explanation.

The papers

Start a renewal file with the participant's full name, MO HealthNet identification number, current mailing address, case number if available, and every notice received. Add the envelope because its postmark may help establish when FSD mailed the notice.

Missouri uses different renewal forms for different eligibility groups. The Missouri DSS forms listing identifies Form FA-402 as the MO HealthNet Annual Renewal for non-MAGI cases and Form IM-1U as the annual renewal for MAGI cases. Do not choose between them from age alone. Ask FSD which form is attached to this case and whether another household member has a separate renewal. Missouri DSS says a household can receive more than one renewal with different due dates. Those agency pages are indexed on the Sources page.

When you contact FSD, ask these questions in order:

What is the renewal due date shown in the case record?
Did FSD complete the renewal from information it already had?
If not, what is the exact form name or form number required?
When and where was the form sent?
What address and phone number are recorded for the participant?
Are verification documents already requested?
Has a closing notice been issued?
What submission routes are available for this case?

If the address is wrong, report the change through the official DSS route and keep the confirmation. An address change and an annual renewal are separate acts. Updating the address does not prove that the renewal was completed.

If the portal displays a renewal, save a copy before you begin. After submission, save the confirmation page and confirmation email. If you upload a paper form or verification documents, keep the complete signed form, every attachment, the upload receipt, and a list of the file names. If you mail the packet, use the address printed on the form and keep a complete copy plus mailing proof. If you deliver it in person, ask for a dated receipt.

Check the signature line before submission. Form FA-402 states that the renewal must be signed. A complete packet without the required signature can still leave the renewal unfinished.

The runway

Build the runway backward from the due date printed by FSD, not from the day you happened to find the letter. Put four checkpoints on one page: due date, submission date, FSD confirmation date, and eligibility decision date.

A portal confirmation proves submission. It does not prove approval. Missouri DSS says FSD sends a letter after reviewing the information and that the portal can show the next renewal date. Keep checking until you have the decision or can see that updated date.

If coverage already shows closed, read the closing notice before starting a new application. Ask FSD whether the missing renewal can still be accepted, whether coverage will be restored during review, what verification remains outstanding, and whether a new application is actually required. Cure periods and effective dates belong to the case record and the notice. Get the answer from FSD rather than estimating it.

At the same time, make a claim list. For each appointment, prescription, ride, supply, or service affected by the closure, record the provider, service date, amount billed, payer response, and resubmission contact. Ask providers to hold the account from collections while eligibility is being reviewed, but get any agreement in writing. The Consumer Financial Protection Bureau materials indexed on the Sources page explain why you should inspect medical bills and collection notices rather than assuming they are accurate.

Do not sign a payment plan merely to stop the calls before you know whether the claim can be rebilled. If a provider demands payment, ask for an itemized bill and the written payer response. The provider handles the claim. FSD handles eligibility. Keep those conversations in separate notes.

Free help

Call the FSD number printed on the renewal or closing notice for the official case answer. Write down the date, the representative's name or identifier, and the exact next action. If the answer is only “wait,” ask what document is pending, when it was received, and how you will learn the decision.

Aging Ahead serves older adults and caregivers in the St. Louis region and can help you locate benefits counseling and local support. Missouri SHIP provides free counseling when the problem also involves Medicare. Their public routes are collected on Local Help With Paying for Care.

If FSD issues an adverse decision, use the hearing instructions and deadline printed on that notice. Keep the notice, renewal, proof of submission, verification documents, and call log together. Free help can explain process and help organize the file. When the dispute turns on legal rights, representation, or what to argue at a hearing, stop there and contact a legal aid program or use the Missouri Bar's lawyer referral resources listed on the Sources page.

Your finish line is not “form sent.” It is a dated eligibility decision, an updated renewal record, and a claim list that tells every provider what can be resubmitted.
