The Medicare Premium Can Rise on an Old Tax Return. Check the Income Line Before the Bill Changes

Some people with Medicare pay an added Part B premium, and the amount is set from tax information already on file, so the leverage is in the paperwork rather than the bill.

Who Pays ·

Most people enrolled in Medicare Part B pay a standard premium. Some pay more under an income-related monthly adjustment amount, which the Social Security Administration sets (SSA, Medicare Premiums). The adjustment is based on modified adjusted gross income that the IRS reports from a tax return filed two years earlier. That lookback is the fact families most often miss, because the return that drives this year's premium may be the one from two years back.

What counts as income for this purpose

The income used is not the monthly deposit. It starts from the adjusted gross income on the tax return and adds certain tax-exempt income, such as some municipal bond interest (IRS). A large withdrawal from a retirement account, the sale of an asset, or an unusually high year of earnings can all appear on the return that SSA later uses. None of those events needs to be a regular pattern to move the premium.

How to check the number

Find the notice SSA sent about the premium adjustment. It states the amount and the tax year it is based on. Compare the income lines on that return with what you expect the reported income to be. Do not guess from bank deposits, since deposits are not the measure. If the figures are unclear, ask a tax preparer or the IRS to confirm which lines feed the calculation.

When a life event changes the picture

SSA allows a request to reduce the adjustment when a life-changing event lowers income, such as the loss of a spouse, retirement, a divorce, or a reduction in work (SSA, Form SSA-44). Gather the proof the form asks for before you file, and keep copies of everything you send. Missouri SHIP counselors can explain how the request works in plain terms, at no charge.

What not to do

Do not assume the premium will fall when your parent stops working, because the lookback year may still carry the higher income. Do not set aside SSA letters, since the notices start timelines for appeals and corrections. If a divorce, an estate, or a transfer of property is part of the income story, that question goes to a lawyer or a tax preparer, not to this desk.
