The Extra Help Application Is Open. Size the Household Before You Count the Money
Separate the people who live under the roof from the people Social Security actually counts before you submit the prescription-cost application.
You have your parent’s bank statements, prescription receipts and a rough total of what everyone in the house earns. Do not add the whole household together yet. Extra Help is a Medicare program for people with limited income and resources, but its application does not treat every person at the address as one financial unit. The first job is to identify whose money belongs on the form.
Local option: New Plan Care gives families in the St. Louis area in-home help so a parent can stay home: companionship, help with bathing and dressing, meals, and overnight care. Call (314) 405-0887.
Who pays
Extra Help can reduce what a person with Medicare pays under Medicare Part D. It applies to drug-plan costs, not the rent, groceries, caregiving hours or medical services billed under Medicare Part A or Part B. Medicare says a person may qualify automatically through full Medicaid, a Medicare Savings Program or Supplemental Security Income. Confirm those records before starting another application. Use the Medicaid card, Social Security notice and any state premium-assistance notice, not family memory. Medicare and Social Security are the controlling sources listed on this desk’s Sources page.
If automatic qualification is not established, Social Security decides the Extra Help application. The income and resource limits can change. Do not copy a limit from an old article or letter. Check the official Extra Help page or call Social Security using the number on the person’s Social Security correspondence. Ask for the limit that applies to the applicant’s marital and living arrangement.
The papers
The application is Form SSA-1020, Application for Extra Help with Medicare Prescription Drug Plan Costs. Start with the marital-status question. If the applicant is married and living with a spouse, the form asks for the spouse and generally counts the couple’s income and resources. If the applicant is unmarried, widowed, or married but not living with the spouse, do not assume that an adult child, sibling or roommate becomes a spouse-equivalent applicant merely because that person shares the address.
Then read every household question on the form. SSA-1020 may ask about relatives who live with the applicant and receive at least half of their support from the applicant or spouse. That question can affect how Social Security evaluates income, but it does not mean you should pour every resident’s wages and accounts into the applicant’s totals. If you cannot tell whether a separation, temporary stay or support arrangement fits the wording, stop and ask Social Security how to answer. That is an agency classification question. If the dispute turns on marital status, ownership or another legal right, take it to a lawyer.
Build two worksheets before entering numbers. On the income sheet, list Social Security, pensions, wages, annuities, veterans benefits and other income requested by the form for each person SSA-1020 tells you to include. On the resource sheet, list the requested bank accounts, investments, retirement accounts and real estate other than the home. Keep excluded items separate instead of assigning them a guessed value. The form itself identifies exclusions, including the principal home, vehicles and personal possessions.
Use statements that cover the same reporting point. Write the owner beside every account and mark joint accounts plainly. Keep the statement supporting each number. Do not subtract debts, care expenses or promised family payments unless the form specifically directs you to do so.
The runway
Before filing, make a one-page control sheet with the applicant’s name, Medicare number, Social Security number, marital and living arrangement, people included, filing method and the documents used. Record the submission confirmation or mailing details. Keep a complete copy of the answers even if you apply online.
SSA-1020 applies for Extra Help. It does not enroll the applicant in a Part D plan. After the decision arrives, confirm that the person has drug coverage and that the plan covers each prescription at the pharmacy the person intends to use. Send medication substitutions or questions about whether a drug should change to the prescriber. Send formulary, pharmacy-network and cost questions to the plan using the number on the member card.
The form can also start the application process for a Medicare Savings Program by sending information to the state unless the applicant declines that referral. Treat that as a handoff, not an approval. Watch for a separate request or decision from Missouri’s Medicaid agency, MO HealthNet Division, and answer it from the same document file. Medicaid.gov, Missouri DSS and Medicare explain these separate payer roles through the public agencies collected on the source list.
When the decision arrives, match the name, household treatment and program result to your control sheet. Keep the notice. If Social Security denies the application or uses facts you believe are wrong, read the appeal instructions immediately. Form SSA-1021 is the Social Security appeal form for an Extra Help determination. Do not replace the appeal with repeated phone calls.
Free help
Missouri SHIP provides free Medicare counseling. Bring the SSA-1020 questions, Medicare card, current drug-plan card, income list, resource list and any decision notice. Ask the counselor to help you separate three questions: whether Extra Help fits, whether a Medicare Savings Program application is also moving, and whether the existing Part D plan still fits the prescriptions and pharmacy. Start with this desk’s local help directory.
Your finish line is not a guessed eligibility answer. It is a filed application built around the correct people, a saved receipt, a tracked state handoff and a decision notice matched to the drug plan that will actually process the prescriptions.
