Mom Moved Into Your House. Does Your Income Count for Medicare Savings Help?

Size the application around Mom and any spouse living with her, not everyone under your roof, then let Missouri decide which Medicare Savings Program fits.

The papers ·

Your mother lives in your house. You buy groceries, cover utilities and may claim her on a tax return. None of that tells you, by itself, whose income belongs on her Missouri Medicare Savings Program application.

Local option: New Plan Care gives families in the St. Louis area in-home help so a parent can stay home: companionship, help with bathing and dressing, meals, and overnight care. Call (314) 405-0887.

For this application, start with the Medicare beneficiary. Missouri DSS says a spouse's income and resources are considered when that spouse lives in the home. It also says eligibility is decided separately for each applicant. That is a different sizing rule from the tax-household rules you may know from other health coverage applications. Your wages do not become Mom's income merely because she moved into your spare room or uses your address. Who pays

Medicare Savings Programs are administered by the state. Depending on the program Missouri approves, the state may pay a Medicare Part A premium, a Part B premium, or certain Medicare deductibles and cost sharing. Medicare identifies four programs: Qualified Medicare Beneficiary, Specified Low-Income Medicare Beneficiary, Qualifying Individual and Qualified Disabled and Working Individual. Missouri DSS uses the shorter labels QMB, SLMB, QI or SLMB2, and QDWI. Source: Medicare, Medicare Savings Programs; Missouri DSS, Medicare Savings Program manual section 1805.065.00.

Do not choose a program by checking a box against a number you found online. Income and resource limits change, Missouri applies its own counting rules, and different programs pay different parts of Medicare. Ask the Family Support Division to screen the application for every Medicare Savings Program available to Mom. For the limits Missouri is applying to her case, call the number printed on the current state application or decision notice. That puts the rate question with the agency making the decision.

If Mom qualifies for a Medicare Savings Program, Medicare says she will also receive Extra Help with Medicare drug costs. That does not mean the two applications, notices and effective dates are interchangeable. Keep each notice in its own file. Source: Medicare, Medicare Savings Programs and Help With Drug Costs.

The papers

Use Missouri form IM-1MSP, Application for Medicare Savings Programs. The form is specifically for help with Medicare premiums. Do not substitute a marketplace application, a tax return or a general letter asking for assistance. Download a fresh copy from Missouri DSS or request one from the Family Support Division so you are answering the version the agency is accepting.

Build the file in this order:

1. Identify the applicant. Put Mom's legal name, address, Social Security number and Medicare information in the applicant fields. If her spouse lives with her, answer the spouse questions and indicate whether the spouse is also applying. Your name belongs in a helper or authorized representative field only if the form provides one and that role is accurate.

2. Draw the household before entering money. Write Mom in the center. Add her spouse if they live together. List other residents separately on your worksheet, including yourself, your spouse and children, but do not pour everyone's income into Mom's column. Missouri DSS says the income and resources of a spouse living in the home are considered for QMB. It does not say an adult child's paycheck is counted simply because parent and child share an address. Source: Missouri DSS, QMB Manual section 0865.020.10.

3. Sort income by owner. Make one line for each payment Mom receives and, if applicable, each payment her live-in spouse receives. Use the gross amount before deductions unless the form instructs otherwise. Match each line to evidence such as a Social Security benefit letter, pension statement, pay record or other payer statement. If you regularly give Mom money or pay bills for her, do not guess how Missouri will classify that support. Describe the arrangement and ask the Family Support Division whether it must be reported and where.

4. Sort resources by ownership. List accounts and property the form asks about under the person who owns them. For a joint account, record every named owner and the balance shown by the statement. Do not quietly divide a balance because you believe only part belongs to Mom. Attach a short explanation and the records showing deposits or ownership, then let the eligibility worker apply the rule. Medicare says states may count items such as money in bank accounts, stocks and bonds, while some property is excluded. Source: Medicare, Medicare Savings Programs.

5. Separate tax dependency from program household size. If you claim Mom as a dependent, keep that fact available because the form or worker may ask about it. Do not treat it as proof that your income counts. Medicare Savings Program eligibility uses rules tied to aged, blind and disabled Medicaid eligibility, not the ordinary tax-household method used for many other Medicaid groups. Source: Medicaid.gov, Eligibility Policy; Missouri DSS, QMB Manual section 0865.020.10.

6. Get the right signature. Mom signs if she can understand and complete the application. A spouse may be permitted to apply, and an authorized representative may act when properly appointed. Living together, paying Mom's bills or being named as an emergency contact does not automatically give you authority to sign her name. If there is doubt about capacity or authority, stop before signing. Ask Missouri DSS what documentation it accepts. If the answer depends on interpreting a power of attorney, guardianship order or other legal instrument, that is the line where a Missouri lawyer takes over.

The runway

Make a copy of the entire signed packet before submission. Keep the application, every attachment and proof of delivery together. On the front, write the applicant's name, the form number IM-1MSP, the submission method and the confirmation or tracking information.

When Missouri asks for more information, answer the exact request. Put Mom's name and case number on each page. Send copies unless the notice specifically requires an original. Keep the request and your response together so you can show what was supplied.

When a decision arrives, compare it with the file rather than reading only approved or denied. Check:

• Which person was evaluated
• Which Medicare Savings Program was considered
• Which income sources and resource balances Missouri used
• Whether a live-in spouse was included
• Whether your income was mistakenly attributed to Mom
• The effective date
• Any instructions for reporting changes or renewing coverage
• The hearing instructions and response deadline if you disagree

If the household was sized incorrectly, call the number on the notice and state the problem plainly: “The applicant lives with her adult child. She does not live with a spouse. Please identify the rule under which the adult child's income was counted.” Take the representative's name, note the call reference number and follow the notice's written appeal instructions if the issue is not corrected. This desk can help you organize the paper trail, but it cannot tell you whether to file a legal appeal or argue a legal interpretation.

Free help

Missouri SHIP provides free, unbiased and confidential Medicare counseling. Its counselors can check for Medicare savings programs and help with applications. The program does not sell insurance. Source: Missouri SHIP, Our Services, with contact routes collected at Local Help With Paying for Care.

Bring the counselor Mom's Medicare card, the current IM-1MSP, income records, recent account statements, any spouse information and every Missouri notice already received. Ask one sizing question first: “Whose income and resources belong in this applicant's eligibility unit?” Then ask the counselor to mark each requested proof against a line on the form.
