Two Medigap Policies Share a Letter. Make the Timing and Rate Method Prove the Choice Before You Apply
Before you commit, confirm the plan letter, enrollment right, rate method, effective date, and policy form number with the insurer itself.
You have two Medicare supplement offers on the desk. Both say Plan G, Plan N, or another letter. One premium looks better. That is not enough to choose.
Local option: New Plan Care gives families in the St. Louis area in-home help so a parent can stay home: companionship, help with bathing and dressing, meals, and overnight care. Call (314) 405-0887.
Medigap policies with the same letter provide the same standardized medical benefits, but the companies can use different rate methods, discounts, underwriting rules, and administrative procedures. Medicare pays first for covered Original Medicare services. The Medigap insurer then pays the portion assigned to that policy. Medigap does not replace Medicare, include prescription drug coverage, or pay for most long-term care. Those boundaries come from Medicare's Medigap guidance, collected on this desk's Sources page.
Your job before applying is to prove five items: which plan letter you are comparing, whether the applicant has a protected right to buy it, how the insurer sets and changes the premium, when coverage will start, and what must happen before any existing coverage ends.
Who pays
Start with the Medicare card and the Medicare account. Confirm that Part A and Part B are active and write down the coverage start information exactly as Medicare shows it. Then identify the present coverage: Original Medicare alone, Original Medicare with an existing Medigap policy, Medicare Advantage, retiree coverage, Medicaid, or another arrangement.
Do not compare a Medigap offer with a Medicare Advantage plan as if they were two versions of the same product. A Medigap policy supplements Original Medicare. It does not work with Medicare Advantage. If the person is leaving Medicare Advantage, ask Medicare or Missouri SHIP which disenrollment action must be completed and when Original Medicare resumes. Do not cancel the current arrangement based on an agent's verbal assurance.
Next, write the exact Medigap letter at the top of each quote. Compare the same letter with the same deductible design. If one offer is Plan G and another is a high-deductible Plan G, they are not the same payment arrangement even though they share a letter.
Ask each insurer these questions by phone:
What is the complete legal name of the underwriting company? What is the policy form number? Is this an individual policy or coverage offered through a membership or group arrangement? Is the rate community-rated, issue-age-rated, or attained-age-rated? Which discounts are included in the quoted premium, and what event can remove each discount? Does the quote assume automatic bank withdrawal, a household arrangement, tobacco status, or another condition? Is any application fee or separate membership charge involved? What is the official premium for this applicant and ZIP code?
The Missouri Department of Commerce and Insurance publishes a Medigap rate-shopping tool, but the department warns that displayed rates are not guaranteed. Use it to find insurers and policy form numbers, then send every rate question to the insurer's telephone representative. Record the representative's name, the call reference number, and the rate the company states for the exact policy form.
The papers
Build one comparison sheet. Give each insurer a column and record the company name, plan letter, deductible design, policy form number, rating method, quoted premium, included discounts, effective date, underwriting status, payment method, and customer-service number.
There is no single federal Medigap application form number used by every insurer. The insurer supplies its own application. That makes the policy form number and application edition important. Put both on your sheet. If the agent cannot identify the policy form being offered, stop the application.
Collect the documents that prove the applicant's enrollment position:
The Medicare card or Medicare account record showing Part A and Part B, any notice explaining why prior coverage is ending, the current policy or plan identification card, the existing Medigap policy if one is being replaced, and every letter that states a right to buy Medigap without medical underwriting.
Medicare calls that protection a guaranteed issue right. The qualifying event, permitted plan choices, proof required, and application window matter. Do not decide from memory. Read the applicable Medicare notice with Missouri SHIP, then ask the insurer to state in writing whether it is processing the application under open enrollment, a guaranteed issue right, or medical underwriting.
Read the health questions before signing if underwriting applies. Answer from the applicant's records. Do not let an agent suggest what can be omitted. If a question is unclear, ask the insurer to explain it in writing. The application should not contain blanks when it is signed.
Ask for the completed application, the outline of coverage, the policy, the company's replacement notice if an old policy is involved, the premium receipt, and written confirmation of the effective date. Keep the envelope or electronic delivery record with the policy.
The runway
Do not cancel an existing Medigap policy when the new application is merely submitted. An application, a quoted premium, and an agent's statement are not proof that coverage is active.
Wait for the new insurer to confirm acceptance, the final premium, the policy form, and the effective date in writing. Then compare that effective date with the termination date proposed for the old coverage. Ask both insurers how premium refunds, automatic withdrawals, and overlapping coverage are handled.
Medicare provides a limited review period after a new Medigap policy is delivered. If you are replacing another Medigap policy, Medicare guidance describes a limited period in which the old policy may be retained while you test the new one, although both premiums may be due. Confirm the exact procedure with Missouri SHIP and both insurers before sending a cancellation request.
Put four follow-ups on the work sheet: confirm the application was received, confirm whether underwriting or proof is still outstanding, confirm the policy was issued with the expected terms, and inspect the first premium withdrawal. If the issued policy differs from the application or quote, call the insurer before the review period expires.
A lower opening premium does not prove a lower future premium. Ask how age affects the rate, which increases can occur for reasons other than age, whether a discount can expire, and where the answers appear in the policy. No representative can promise future rates. You are comparing the rate method and written conditions, not predicting a price.
Free help
Missouri SHIP provides free, insurance-neutral Medicare counseling. Take the Medicare card, both quotes, the present coverage documents, and any termination notice to the counseling call. Ask the counselor to verify the plan letters, the applicant's enrollment or guaranteed issue position, and the order for changing coverage. Contact information is collected on Local Help With Paying for Care.
For licensing, policy-form, rate-filing, or complaint questions, call the Missouri Department of Commerce and Insurance. For Medicare enrollment status and the interaction between Original Medicare and Medicare Advantage, call Medicare. Make each office answer only the part it controls.
If the disagreement is about what the application, replacement notice, or issued contract legally requires, stop at the contract line. A Missouri Bar lawyer-referral resource can help you locate counsel. This desk can help you organize the payer file, but it cannot interpret a disputed contract or tell you what legal claim to make.
The commitment point is simple: do not submit the application until you can name the payer arrangement, plan letter, policy form, enrollment right, rate method, and requested effective date. Do not end existing coverage until the new insurer has issued the policy and the transition has been checked with the offices responsible for it.
