Set the Medigap Letters Side by Side Before the Agent Calls Back

A Medigap plan is chosen by its standardized letter and benefit grid, and it does not pay for in-home aides, so compare the letters on paper first.

Who Pays ·

Medigap, also called Medicare Supplement insurance, works alongside Original Medicare. It helps pay some of the costs Medicare leaves behind, such as deductibles and coinsurance on covered services. It does not pay for in-home aides, help with bathing or dressing over the long term, or prescription drugs, which a separate Part D plan handles. Medicare describes standardized Medigap plans by letter, and each letter promises the same core benefits wherever it is sold. Use the letter, not the brand name or the agent's summary, as the unit of comparison.

Local option: New Plan Care gives families in the St. Louis area in-home help so a parent can stay home: companionship, help with bathing and dressing, meals, and overnight care. Call (314) 405-0887.

Build the grid before the call

Pull the standardized benefit grid from Medicare for each letter you are considering. Write down, one letter per row, which cost-sharing items that letter covers in full, in part, or not at all. Then mark which of those items your parent actually faces based on their own Medicare statements from the past year of care. A plan that covers a cost your parent never incurs is paying for a risk that is not theirs.

Ask about the rating method

Medigap premiums are set by one of a small number of rating methods, and Medicare explains them in its consumer materials. Ask the agent which method the policy uses, and get the premium and the method in writing for your parent's age and location. Ask whether the premium can change over time and under what conditions. Do not accept a verbal number as the basis for a decision.

Check the rules that can block a switch

Ask whether the insurer uses medical underwriting for your parent, and under what circumstances. Ask whether the policy can be bought outside an open enrollment window. Confirm that the company and agent are licensed to sell in Missouri. Medigap cannot be held together with a Medicare Advantage plan, so if the offer comes with a suggestion to drop Original Medicare, get that suggestion explained in writing before you act on it.

Get a second read that does not sell anything

Missouri SHIP offers free counseling on Medicare choices, and its counselors do not earn a commission on the plan they help you compare. Take the grid, the written quote, and the list of your parent's usual providers to that session. You can find the local options on the Local Help With Paying for Care page. For how this desk checks its own claims, see How We Write.

Where this stops

Medigap answers the question of Medicare's cost-sharing. It does not answer the question of who pays for long-term custodial help at home. For that, your parent's income, assets, and any Medicaid eligibility questions belong with the Missouri programs and, where assets are involved, with a lawyer. Your parent's doctor decides what care is medically needed.
